need to
1. First complete either one of the following two order forms.

- A plain text order which you print out from your browser and fill in
by hand is

2. Decide if you want to include your credit card information on
the order form or call it in.
(If you wish to telephone your credit card information, indicate
that your will do this in the comments section of the order form and fax
it. Otherwise include your credit card information on the form and
fax it. If you will telephone us your credit card information,
remember to fax your order first before you call us.)
3. FAX your completed order form (with or without credit card
information to 1-512-388-3974 - FAX.
4. If necessary to provide your card information, telephone us
